order to validate your Connected Equipment Warranty, you must
complete this form within 20 days of purchase. You can also fill
out and mail your Warranty Registration card that came inside the
box of the product you purchased.

- If the Smart Power Systems equipment fails and this failure causes the surges to pass through and
damage the connected equipment, Smart Power Systems will pay for the
repair or replacement of the connected equipment in accordance to Connected Equipment Protection
